  1. HSV-1/2
    • DS DNA virus
    • Nuclear envelope
    • LAT-1 - prevents alpha-0 action which causes adecrease in transcription
    • Beta genes: enzymes
    • Gamma genes: strutural proteins
    • Transmission: direct contact
    • HSV 1: gingivostomatitis (kids 1-6), labialis, keratoconjuctivitis, encephalitis, eczema
    • HSV-2: vulvovaginitis, progenitalis, asceptic meningitis
    • Neonatal herpes: high mortality rate
    • Dx: clinical, IF, viral isolation
  2. VZV
    • Chicken pox: primary infection
    • inhalation
    • incubation: 14-18 days
    • mild fever, malaise, headaches, vesicles
    • transformation possible
    • Shingles: reactivation
    • 50% over 50
    • pain before vesicles
    • Dx: clinical, IF
    • prevention: live vaccine
    • Tx: supportive, acyclovir
  3. CMV
    • Nuclear, cytoplasmic inclusion bodies, giant cells
    • Congenital: hearing loss, mental retardation, junadice, organomegaly, anemia, low eight, microcephaly
    • Imunnosuppressed: CMV pnuemonia
    • BM grafts: CMV pnuemonia leading cause of death
    • HIV: retinitis, gastroenteritis
    • Transmission: cmose contact
    • Dx: isolation, EM, PCR
    • Heterophil negative mono
    • tx: gancyclovir, IVIG
    • prevention: vaccine
  4. EBV
    • Heterophil positive mono (infectious mono)
    • African burkitt's lymphoma
    • EBNAs, VCA: viral markers
    • Transmission: contact
    • Disease: low fever, headache, sorethroat, fatigue, night chills, enlarged spleen, atypical lymphocytes
    • Complications: laryngeal obstruction, meningitis, splenic rupture
    • Dx: CBC (10% downey cells), heterophil positive Ab, VCA Ab
    • Tx: acyclovir
