Indications for PROM
- The Pt has acute inflamed tissue
- The Pt is unable to or not supposed to actively move a segment
-
Goals for PROM
- Maintain joint and connective tissue mobiliyt
- Minimize the formation of contructures
- Maintain elasticity of muscle
- Assist circulation and vascular dynamics
- Ennhance synovial fluid movement
- Devrease or inhibit pain - assist with healing
- Help maintain Pt awareness of movement
- To assess or demonstrate
-
Indiecations and Goals for AAROM
- Same as PROM with added benefit of muscle contraction
- Sensory feedback
- Increase circulation
- develops coordination / motor skills for function
- can increase strength in poor / fair range
-
Goals for AROM
- maintain elestivity and contractility of muscles
- Provide sensory feedback from the contracting muscles
- Provide a stimulus form bone and joint integrity
- Increase circulation and prevent thrombus formation
- develop coordination and motor skills for functional activites
-
Indications for AROM
- Whena pt is able to actively contract muscles
- when a pt has weak muscles
- for low level aerobic conditioning - warm up/ cool down
- to mobilize segment above and below and immobilized region
-
Precautions / Contraindications
- When motion is distruptive to the healing preocess - should be pain-free and should not increase inflammtion
- When pt respone or the condition is life threatening
-
What does the PTA need to do to apply ROM
- Adequately suppor and stabilize
- Move the segment throught its painfree ROM just to the point of reistance, then return to starting posistion
- Motion should be SLOW, smootha nd rhythmical
- DO NOT force motion, the goals is not to stretch
-
Options for ROM
- straight plane (anatomical planes)
- Diagonal patterns
- Functional patterns
- Fuange of muslce elongations for 2 jointed muscles
-
What is the difference between ROM ex and flexibiltiy Ex?
ROM is intended to provide motion not strectching
-
AAROM equipment
- Wand or T-bar
- Finger ladder, wall climbing, ball rolling
- Overhead pulleys
- skate board/ powder board
- Recirocal exercise unit
-
CPM machine
- Passive motion taht is performed by a mechanical device taht moves a joint slowly and continuously through a pre-set controlled ROM
- Very usefull for total joint surgery
- Needs to be carefully monitored for alignment
- can increase pain and inflammation if not used properly
- need to work with nursing staff to optimize the benefits
-
What to include in documentation of ROM exercise
- What type of ROM
- What position
- What type of assistance provided if any
- Left right or bilateral
- number of reps
- diagonals, straight plane, functional movement or range or muscle elongation
- Quality of movement (if apporpriate)
